Please fill and send the form which is addressed to you.

FORM BY E-MAIL, BY MAIL OR BY FAX

1. Fill, print and return the rental on a temporary basis form, completed a (1) month before the scheduled dates.   

2. Include in your sending a $1,000,000.00 insurance proof of civil liability (photocopy of your insurance policy).

3. Include in your sending, a copy of your Trading License (applies to non-profit organizations only).

4. Include in your sending, a copy of a resolution giving you the right to sign for your organization.
   
   
5. Send to us these three documents with your requestone month before the dates desired:

6. After the analysis of your demand, a written communication will be sent to you for your rental on a temporary basis.
   
   Your request is effective as from the moment when we receive this printed form and the copy of the documents requested by e-mail, by fax or by mail.
